TERMS AND CONDITIONS

Please read carefully. All blank areas will be filled-in on the original contractual document during consultation.

1. The parties herein agree as follows: the initial term of this contract shall be from _______________________ through _______________________.

2. In the event of early return home, client must promptly notify Stay and Play at Home @ 423-335-5128 to avoid being charged for unnecessary visit(s).

3. The fee is ( ________ ) x ( ________ visits) for a total of __________________________. Other fees for additional services or circumstances may apply. Any additional visit(s) made or service(s) performed shall be paid for at the agreed contract rate. Stay and Play at Home is authorized to perform care and services as outlined on this contract. Stay and Play at Home is also authorized by Client (name entered below) to seek emergency veterinary care with release from all liabilities related to transportation, treatment, and expense.

4. Should specified veterinarian be unavailable, Stay and Play at Home is authorized to approve medical and/or emergency treatment (excluding euthanasia) as recommended by a veterinarian. Client agrees to reimburse Stay and Play at Home for expenses incurred, plus any additional fee for attending to this need or any expenses incurred for any other home/food/supplies needed.

5. In the event of inclement weather or natural disaster, Stay and Play at Home is entrusted to use best judgment in caring for pet(s) and home. Stay and Play at Home will be held harmless for consequences related to such decisions.

6. Stay and Play at Home agrees to provide the services stated in this contract in a reliable, caring and trustworthy manner. Inconsideration of these services and as an express condition thereof, the client expressly waives and relinquishes any and all claims against Stay and Play at Home except those arising from negligence or willful misconduct.

7. Client understands this contract also serves as an invoice and takes full responsibility for PROMPT payment of fees upon completion of services contracted. A finance charge of 10% per month will be added to unpaid balances after thirty (30) days. A handling fee of $35.00 will be charged on all returned checks. One half deposit is required on lengthy assignments. First time clients or clients with a history of late payment(s) will be required to pay in advance before services are rendered. Stay and Play at Home accepts cash and personal checks. In the event it is necessary to initiate collection proceedings on the account, Client will be responsible for all attorney's fees and costs of collection.

8. In the event of personal emergency or illness of your primary Stay and Play at Home pet care provider/sitter, Client authorizes the arrangement for a secondary Stay and Play at Home pet care provider/sitter to fulfill responsibilities as set forth in this contract. Client will be notified in such a case, if time and circumstances allow.

9. All pets should be currently vaccinated. If any Stay and Play at Home personnel is bitten or otherwise exposed to any disease or ailment received from Client's animal which has not been properly and currently vaccinated, it will be the client's responsibility to pay all costs and damages incurred by the victim.

10. Stay and Play at Home reserves the right to terminate this contract at any time before or during its term if it is determined that Client's pet poses a danger to health or safety of himself/herself or others. Especially if these concerns prohibit pet sitter from caring for pet, Client authorizes pet to be placed in a kennel, with all charges from there to be charged to client.

11. Client authorizes this signed contract to be valid approval for future pet care services, permitting Stay and Play at Home to accept reservations for pet care services and enter the premises without additional signed contracts or written authorization.
